The pack explains the provision, the agreed session times (morning 9:30 – 11:30, afternoon 12:30 – 14:30), what we need from you, and includes the referral form to complete and attach below.

Step 2

Submit the referral with documents

Attach the completed referral form, the latest academic report, the EHCP and PEP where they exist, and a risk assessment. Documents are stored securely in Kloodle Hub and the whole Legacy team is notified the moment you submit.
